- 1 of the following documents containing the person's date of birth and place of birth
- Hospital birth record or certified copy thereof
- Grana Niladhari Report or its certified copy
- Estate Birth Record or certified copy thereof
- A copy of the Family Health Officer's birth certificate
- 1 of the following documents if the above documents are not available
- Health Growth Chart
- Baptismal certificate
- A certified copy of the first school admission document

- A valid ID
- If parents are married, marriage certificate (if unmarried, if father accepts paternity, father's affidavit and signatures of mother and father in column 10 of L.P.A. 6 declaration form)

Delayed registration of deaths
- Objectives
- Registration of death of a person whose death is confirmed to be unregistered.
- Required Qualification/Eligibility
- Where the place of death is situated within his division.
- Where the place of death is situated within his division.Confirmation that death has not been registered.
- Necessary documents
- Declaration under section 36.Declaration under section 36.
- A document search results in a death not being registered.
- 1 of the following documents containing the date and place of the person's death
- Hospital report or its certified copy
- Grama Niladhari Report or its certified copy
- Estate death certificate or certified copy thereof
- Obituary
- A letter certified by the concerned Clergy regarding participation in religious activities.
- Bills from funeral service providers.
- Letters issued by Welfare Societies regarding the payment of gratuity for the death of the concerned.
- Documents to prove the age of the deceased.
- Documents to prove if the deceased was employed.
- Medical report confirming cause of death.
- Affidavit.
- Grama Niladhari Certificate.
